History: The patient is a 27-year-old, right-handed female. Two weeks ago on the 19th of May she was at the gym. She began feeling nauseous and nearly vomited. She walked out to get some air but noticed spots before her eyes, her vision seemed blurred, the room color seemed different and she developed a right-sided headache. She took a break, walked over to the student health clinic, went home and after a nap all of her symptoms had resolved with no medications or interventions. She has never had a previous episode such as this and no problems since.

She does have “regular headaches” frequently that are mainly a tension type headache in the base of her neck that can last days at a time but generally do not require medications. She uses an orthopedic pillow and massage.

In 2008 she was involved in a motor vehicle accident in which her vehicle was rear-ended. She apparently suffered a concussion at the time, was seen at XXX where CAT scans and subsequent MRI was negative. She then flew back to her home in Houston and had been followed by Dr. XXX and physical therapy for a year or so until she was released. She complains of being tired during the day, falling asleep during class, forgetting a lot, repeating herself. She gets lost. She switches words typing, has word recognition and spelling problems and cannot multitask. If she overworks her brain she says her brain “stops connecting” and she has to nap. She is however completing her PhD in Literature and works as a counselor [sic].
